The Monument consisted of a simple plaque that was made by sculptor Gazzeri with the financial contribution of the people of Forano and later modified with the names of new fallen soldiers from the African War and World War II.

The Monument was initially located in the square in front of the Church of Santa Maria del Colle (today Piazza V. Emanuele) and consisted of a simple plaque carved with the names of the Fallen.
The work was created by the sculptor Gazzeri with the financial contribution of the inhabitants of Forano and subsequently modified with the names of the new fallen of the African War and the Second World War.
After the church was demolished to allow the construction of the headquarters of the Rieti Savings Bank (1969), the plaque was inserted in the Monument created by the artist Vannucci of Terni and moved to the Parco della Rimembranza, next to the Town Hall.
